Full time employees, who work 30 hours per week or more, become eligible for group insurance benefits on the first of the month following six months of employment. For employees who terminate employment while covered by TSLI/HHB's group insurance, coverage ends on the last day of the month in which they terminate. A Summary Plan Description (SPD) for each benefit is distributed to employees at the time of insurance enrollment. In addition, the medical health care plan SPD is distributed annually. Refer to the Personnel Policies for information regarding coverage during medical leave or when an employee reduces the number of hours worked.
 
Employee Contribution
The two options for the Medical Plan are Emblen HIP Acess II and HIP POS. Employee pays up to 24% of the cost of medical insurance.
 
Dental, vision and life insurance are provided at no cost to employees.

Tuition Benefits: Personnel Reimbursement for Education Program (PREP)
PREP provides tuition reimbursement for all full time employees with an overall rating of "Meets Agency Expectations" on his/her evaluation. Refer to Personnel Policies for benefit amounts. Courses must directly relate to the position held by the employee and must be approved by the Chief Executive Officer. All graduate level courses that are approved for reimbursement under PREP must be taxed. Reimbursement for undergraduate courses will not be taxed if the courses are directly related to the position held by the employee.
 
Child Care Benefits
Full time employees who pay for child care during their work hours become eligible for partial childcare reimbursement after six months of employment. Reimbursement is available only for children age 12 and under.
·         $1/hour up to $45/week; one child (could amount to over $2,000/year);
·         $.13/hour up to $5.85/week; each additional child.
·         Chldcare3 reimbursement is included on your W-2 as dependent care benefits.
 
Flexible Spending Account (FSA)
The Flexible Spending Account (FSA) Plan for Medical Care Reimbursement allows you to pay for certain medical and/or dependent care expenses with tax-free dollars through your bi-weekly payroll.
Medical Care Reimbursement allows you to pay for un-reimbursed health, dental or vision expenses as determined by the Internal Revenue Service. Dependent Care Reimbursement allows you to pay for un-reimbursed care for children up to age 13, handicapped children or adults or elderly individuals who rely on you for financial support and care and are eligible to be claimed as an exemption on your federal tax return. Full time and part time employees may enroll in the FSA after the waiting period, which is six calendar months from the first of the month following date of hire. Employees may also enroll during the period of open enrollment, which is the month of December, each year.

The 401(k) Plan allows employees to invest pre or post tax dollars for retirement through payroll deduction. Employees may choose to enroll in a Traditional or Roth 401(k).  Eligibility requirements for employees: 21 Years Old; Employed at least one year; Work at least 1000 hours during each year employed. See the TSLI/HHB 401(l) Summary Plan Description for additional information.

Sick Time Incentive
Full time employees who do not use sick time during a specific six-month period (January 1 to June 30 and July 1 to December 31) and are employed from the first day of the period and for the entire period are eligible to receive $200; part time employees receive $100.
 
Referral Incentive
On a quarterly basis the Personnel Department distributes a Staff Recruitment Award Memo to the management team requesting that staff refer qualified applicants to the Agency. If the referred candidate is hired and completes six months of employment the staff member receives a $250.00 recruitment incentive. Staff members that refer a Per Diem Worker who completes 200 hours of per diem work during their first six months and are still actively working receive a $25.00 recruitment incentive.
