TSLI Pathways is a community residence
program serving psychiatrically disabled adults in Nassau and
Suffolk Counties, New York. TSLI Pathways operates under the
community residence model defined by the New York State Office of
Mental Health. Under this model, the program is made up of
two components; the congregate living Supervised Program and
the more independent Apartment Program. This model enables residents to progress at their own
individual skill level with no pre-determined length of stay in
either program.
In the Supervised Program there is
twenty-four hour staff supervision. The level of staff support is
gradually decreased in accordance with the resident's increased
level of functioning and independence in the Apartment
Program.
The goal of Pathways is to restore the
resident's productivity and independence in the community, to
eliminate or significantly reduce the frequency and duration of
the resident's psychiatric hospitalizations and to help the client
realize his/her potential in terms of educational/vocational
goals.
Pathways staff provide rehabilitation services
on site and refer clients to other services that are available
in the community:
